E-cigarettes may damage neural stem cells important to brain function

Puffing on electronic cigarettes can damage neural stem cells important to brain function, a new study says.

E-cigarettes produce a stress response in neural stem cells, researchers at the University of California at Riverside reported in a study published in the interdisciplinary open-access journal iScience.

E-cigarette users may think they’re safer and cleaner than tobacco cigarettes — but evidence is mounting that nicotine is harmful whether it’s smoked in a traditional cigarette or vaped in an e-cigarette. Another recent study found that certain e-cigarette flavorings damage cardiovascular cells.

Such products ‘are not harmless’, said Atena Zahedi, who earned her PhD in bio-engineering and co-authored the paper.

‘Even short-term exposure can stress cells in a manner that may lead, with chronic use, to cell death or disease. Our observations are likely to pertain to any product containing nicotine,’ Zahedi said in a statement.

E-cigarettes set off a complex series of cellular-level events that damage stem cells’ DNA, the researchers said.

‘The neural stem cells get damaged and could eventually die,’ Zahedi said. ‘If that happens, no more specialised cells — astrocytes and neurons, for example — can be produced from stem cells.’

Those cells are critical, the researchers said. Young people and foetuses are especially prone to stem cell damage because their brains are still developing, the researchers said.

That means young people and pregnant women could be particularly vulnerable to harm from e-cigarettes, the researchers say.

‘Their brains are in a critical developmental stage,’ said Prue Talbot, a professor in the Department of Molecular, Cell and Systems Biology who led the research and directs the Riverside campus’ Stem Cell Center.

‘Nicotine exposure during prenatal or adolescent development can affect the brain in multiple ways that may impair memory, learning, and cognition,’ Talbot said in the statement. ‘Furthermore, addiction and dependence on nicotine in youth are pressing concerns. It’s worth stressing that it is nicotine that is doing damage to neural stem cells and their mitochondria.’

Given the prevalence and availability of nicotine in liquid, inhabitable form, Talbot added, ‘We should be concerned about this.’

Revolutionary Method of Generating Stem Cells Through Blood

Stem-Cell

Collaborators from the Queen’s University Belfast and King’s College London (KCL) discovered a treatment method to cardiovascular diseases caused by diabetes.

These scientists have developed an approach in using a small blood sample in producing large quantities of stem cells in a short period of time. They discovered that damaged cells within blood vessels can be generated and replaced by these stem cells. Complications related to the blood vessels can be prevented through this treatment. Some of these include kidney disease, heart attacks, blindness, and amputations in individuals with diabetes. They published their findings in the journal Stem Cells.

Dr. Andriana Margariti, lead author from the Queen’s University Belfast reiterates, “Being able to produce large quantities of stem cells from a few millilitres of blood in a short timeframe is truly ground-breaking. This could revolutionise how we treat a vast number of blood vessel diseases.”

“Previously, this cell transformation process would have involved a skin biopsy, or large volumes of blood, which simply isn’t viable for many patients as it is a risky process which can take a long recovery time.”

“This study focused on stem cells for vascular diseases but the same process can be used to produce stem cells for a number of organs, including the brain and kidneys, which has huge implications for the future of healthcare.”

A specific gene called Endothelial Specific Molecule 1 (ESM1) controls the production and function of newly-generating endothelial cells. Activating this gene holds a potential in regulating different vascular diseases.

The blood vessels are protected by the endothelial cells. Endothelial cells act as protective layer as they arefound on top layer of cells in blood vessels. Cardiovascular diseases cause these cells to be damaged and is most often accelerated in patients with diabetes.

The damage in endothelial cells make people with diabetes and cardiovasculat disease prone to blindness, heart attacks, and poor circulation.

“A major source of mortality among those with cardiovascular diseases, and especially patients with diabetes, is due to irreversible damage to their endothelial cells which can lead to blockage of blood flow to the heart, eyes, kidney and limbs,” Dr. Margariti emphasized.

“One in every two people with diabetes will die from a heart attack. Current treatment for diabetes is often limited to drugs that regulate sugars and fats in the blood, and hypertension, but unless the endothelial cells are repaired, unfortunately, the illness will continue to progress.”

Their study demonstrated that ESM1 gene-expressing stem cells significantly increased the blood flow and has regenerative potential in damaged blood vessels.

In a concept known as stem cell therapy, damage can be repaired through the transplantation of healthy endothelial cells. Professor Alan Stitt, Dean of Innovation and Education at Queen’s University Belfast and co-author explained: “Through the technology developed, we can readily produce stem cells to transplant to damaged blood vessels. We have discovered that activating the particular gene ESM1 will improve the production and function of endothelial cells, reversing the damaged cells. This is life-changing as the results have shown that repairing these cells can stop the progressive illnesses, which will prevent blindness and amputations,” as reported in Medical Xpress.

“Cell transplantation has huge potential though it is not suitable for all vascular diseases such as coronary disease. Now we know how to generate and improve the function of these cells, we will focus on screening drugs to see which treatments will further improve the function of these cells and ultimately improve the lives of millions of people afflicted with these illnesses.”

Stem Cell Therapy and Spine Care

Spine experts, Jeffrey C. Wang, MD, and Zorica Buser, PhD, shed light on the science—and stigma—of stem cell therapy.

Stem cell therapy shows major potential for spine care, and medical practices offering this therapy are reporting a surge of interest from patients. With the promise of regenerating spine tissues affected by degenerative disc disease, stem cell therapy might be the non-surgical solution to back and neck pain.

But, those big claims don’t currently have a strong evidence base to support them. If you’re weighing the risks and benefits of this enticing, new treatment for spine pain, it’s important to learn as much as you can about it.

To clarify the current state of stem cell therapy for spinal disorders, SpineUniverse reached out to Dr. Jeffrey C. Wang, Professor of Orthopaedic Surgery and Neurosurgery at the USC Spine Center in Los Angeles, and Dr. Zorica Buser, Assistant Professor of Research in the Department of Orthopaedic Surgery at the University of Southern California in Los Angeles.

In this two-part series, Dr. Wang and Dr. Buser detail what you need to know about the current state of stem cell therapy for spinal disorders. Part 1 begins the science of stem cell therapy.

How does stem cell therapy work?

Drs. Wang and Buser: Stem cells are undifferentiated cells that can divide and renew for a long period of time, and with the right stimuli they can differentiate (develop) into various specialized cells. Furthermore, research has shown that stem cells have anti-inflammatory properties.

The primary role of stem cells in a cell-based therapy is to replace damaged cells to stimulate tissue regeneration. In the degenerative spine, stem cells could aid by decreasing inflammation, developing into bone or disc cells, and by replacing damaged tissue with new matrix. For example, bone marrow stem cells may aid in bone formation. Ultimately, we strive to use stem cells to replace damaged cells, such as in a spinal cord injury, and hopefully repair or regenerate the nerves.

Are stem cells the only type of cells used in this therapy?

Drs. Wang and Buser: No. In the spine field, two other types of cells called osteoblasts and chondrocytes have been used. However, the clinical evidence and basic research on those cells are lacking. On the other hand, bone marrow aspirates that contain cells and growth factors are currently used in spinal fusion.

What types of spine problems are reportedly helped by stem cell therapy?

Drs. Wang and Buser: There is a lack of evidence and science to match a stem cell therapy to a specific spinal disorder. However, certain clinical trials and health care facilities are using stem cells to treat low back pain and disc degeneration, and during spinal fusion surgery.

Is stem cell therapy approved by the U.S. Food and Drug Administration (FDA)?

Drs. Wang and Buser: No. The evidence is not yet available to gain FDA approval.

Is the use of a patient’s own stem cells considered an off-label use of this biological technology?

Drs. Wang and Buser: For a product to be used off label, it must be FDA-approved for initial indication. As the use of patient’s own stem cell is not regulated by the FDA, it is unclear if it is considered off-label use. However, we are not aware of on-label uses of stem cells for spinal regeneration.

Without FDA approval, insurance will not cover the treatment. What may treatment cost? And, does treatment require multiple visits?

Drs. Wang and Buser: The costs vary, and there is no standardized protocol for the number of treatments because the clinical evidence is lacking. Other factors that affect cost and treatment are the specific spinal disorder and number of spinal levels impacted. Some patients might require one injection, and some might need several visits.

Currently, most patients are paying cash for the injections, so some physicians and health care facilities are capitalizing on this trend. Although we cannot speak for any particular facilities, some are making a lot of money by charging cash—and patients are willing to pay.

Stem Cell – It is a Challenge for the Present and the Future

Low cost stem cell treatment in India
Stem cell research: The biological therapy offered by stem cells is likely to revolutionise the entire health care delivery. Is Malta in time to catch up with this evolution?

The regenerative capability of a living creature was recorded as early as 330 BC when Aristotle observed that a lizard could grow back the lost tip of its tail. Since then there have been slow but steady developments in understanding the regenerative capabilities of human beings.

In the last decade, there has been an information explosion in the area of stem cell research. The biological therapy offered by stem cells is likely to revolutionise the entire health care delivery. Is Malta in time to catch up with this evolution?

Human stem cells have raised hopes in the eyes of healthcare professionals and patients seeking repair of injuries to a variety of organs damaged by serious illnesses which are often considered incurable or irreversible. Stem cells are special cells that could develop into blood, brain, bones and other body organs with the potential to repair, restore, replace or regenerate damaged cells. The hope is that such regenerative therapy would reduce associated morbidity and mortality rates.

The clinical therapeutic benefit is evident in the use of blood stem cells to treat diseases of the blood, a therapy that has saved the lives of thousands of patients with leukaemia. Stem cells are also used for tissue grafts to treat diseases or injury to the bone, skin and surface of the eye. The potential of clinical applications for stem cells to treat everything from arthritis to Alzheimer’s disease has emerged but very few stem cell treatments have been proven safe and effective by regulatory authorities.

All medical treatments have benefits and risks. The process by which science becomes medicine is designed to minimise harm and maximise effectiveness. There is a multi-step process involved in responsibly translating science into safe and effective medical treatments.

During this process, scientists may discover that an approach that seemed promising in the laboratory does not work in animals or that an approach that worked in animals does not work in humans. It may be discovered that a treatment effectively addresses symptoms of a disease or injury in humans but carries unacceptable risks.

In 2017, the European Medicines Agency approved the first stem cell therapy made out of the patient’s own stem cells. It is used to treat one of the complications of Crohn’s Disease. Before a stem cell therapy is approved, the agency considers the possible risks of tumour development and rejection of the stem cells, together with the benefit to the patient.

Stem cell therapy is no longer science fiction. It is a challenge for the present and the future

The Malta Medicines Authority has in recent years built up an excellent reputation in the regulatory sciences. The first task undertaken by the authority since 2013 has been to ensure its sustainability while at the same time retaining its prime task of safeguarding the quality, safety and efficacy of medicines for patients in the European framework. The authority moved on to ensure accessibility to medicines by establishing the Medicines Intelligence and Access Unit.

The time has come for our outstanding professionals in the authority to take on innovative tasks involving new and advanced therapies. These are being spearheaded by the Directorate for Advanced Scientific Initiatives, which is supporting the promotion of science as in the case of the regulation of cannabis for medicinal use.

The key to success is linked invariably to research and education. The medicines authority is a pioneer in encouraging its team to further their studies and research to master and doctorate levels. Professionals are performing groundbreaking research, such as in the dissertation ‘Therapeutic and Economic Implications of Regulating Stem Cell Therapy and Blood Components’. This augurs well for the authority to take up the Prime Minister’s appeal to the nation not to rely on present successes but to move forward towards innovative and challenging endeavours, such as was done in the case of blockchain and the introduction of cannabis for medicinal use.

Innovative initiatives need to be picked up in the early stages to lay a strong foundation for success to be endowed to the next generation, that is, for our children and grandchildren. These investments, which will bear fruit in the future, can only be carried out because in the last five years the government has set a strong economic and intellectual basis.

The authority is a prime example of an entity that has responded to the Prime Minister’s impetus and is moving towards becoming a sustainable and successful entity, especially through substantial investment in its team of highly committed and motivated professionals.

The assignment of the stem cell initiative to the Malta Medicines Authority, by the Secretariat for Consumer Rights, Public Cleansing and Support for the Capital City, was a move in the right direction, signalling a long but virtuous path towards innovation in advanced therapies, which is tomorrow’s science of medicine. Scientific innovation in stem cells cannot be conceived without advanced regulatory science structures and education.

This is something I have explained to the leaders of our tertiary education systems and it is good to see that there are already recommendations to the Senate of the University of Malta to move diligently but expediently in the field of the regulatory sciences.

Stem cell therapy is no longer science fiction. It is a challenge for the present and the future. Challenges present risks. The key to risk reduction in introducing stem cell therapy is the application of robust regulatory sciences.

This is what I am continuously encouraging the Malta Medicines Authority to execute, and I am fully confident that success will be achieved in this area. There are several barriers that still need to be overcome before this novel therapy can be translated from the laboratory to the clinic. But stem cell therapy has brought in optimistic hope among researchers, doctors and patients who are the chief beneficiaries of this innovation.

Adult stem cell derived kidney on a chip

Enter a capThe 3-lane OrganoPlate comprises 40 microfluidic chips embedded in a standard microtiter plate formattion

Scientists from the Hubrecht Institute, the University of Utrecht, and Mimetas have successfully grown kidney tubules derived from human kidney adult stem cells in microfluidic chips.

Henriette Lanz, Ph.D., Director of Biology at Mimetas explains: “Scientists at the Hubrecht Institute managed to reprogram stem cells found in the human kidney cortex to cells that are similar to tubular cells of the kidney. From these cells, my team has grown three-dimensional perfused kidney tubules. We have discovered that these tubules are fully polarised, meaning that they distinguish inside from outside, just like in a real kidney. Moreover, the biologically important barrier function of the kidney tubes is intact. We show that transporter activity is functional, which is a hallmark of kidney tissue, responsible for pumping of nutrients and toxicants across the kidney barrier. We can grow 40 of such tubules in one single OrganoPlate®. The kidney tubes can be used to create disease models that allow the development of novel therapies. This novel technology offers a powerful human alternative to animal testing for toxic side effects of new medicines.”

A mini kidney from the lab doesn’t look like a normal kidney, but the simple cell structures share many of the characteristics of real kidneys. Researchers can use them to study certain kidney diseases. “We can use these mini kidneys to model various disorders: hereditary kidney diseases, infections, and cancer. This allows us to study in detail what exactly is going wrong,” says Hans Clevers, Professor of Molecular Genetics at Utrecht University and the University Medical Center Utrecht, and group leader at the Hubrecht Institute. “This helps us to understand the workings of healthy kidneys better, and hopefully, in the future, we will be able to develop treatments for kidney disorders.”

Can we repair the brain? The promise of stem cell technologies for treating Parkinson’s disease

Can we repair the brain? The promise of stem cell technologies for treating parkinson's diseaseSections of rat brain transplanted with human cells in a preclinical model of PD are being prepared for analysis. Credit: Claire Henchcliffe

Cell replacement may play an increasing role in alleviating the motor symptoms of Parkinson’s disease (PD) in future. Writing in a special supplement to the Journal of Parkinson’s Disease, experts describe how newly developed stem cell technologies could be used to treat the disease and discuss the great promise, as well as the significant challenges, of stem cell treatment.

The most common PD
treatment today is based on enhancing the activity of the nigro-striatal pathway in the brain with dopamine-modulating therapies, thereby increasing striatal dopamine levels and improving motor impairment associated with the disease. However, this treatment has significant long-term limitations and side effects. Stem cell technologies show promise for treating PD and may play an increasing role in alleviating at least the motor symptoms, if not others, in the decades to come.

“We are in desperate need of a better way of helping people with PD. It is on the increase worldwide. There is still no cure, and medications only go part way to fully treat incoordination and movement problems,” explained co-authors Claire Henchcliffe, MD, DPhil, from the Department of Neurology, Weill Cornell Medical College, and Department of Neurosurgery, Memorial Sloan Kettering Cancer Center, New York, NY, USA; and Malin Parmar, Ph.D., from the Wallenberg Neuroscience Center and Lund Stem Cell Center, Lund University, Lund, Sweden. “If successful, using stem cells as a source of transplantable dopamine-producing nerve cells could revolutionize care of the PD patient in the future. A single surgery could potentially provide a transplant that would last throughout a patient’s lifespan, reducing or altogether avoiding the need for dopamine-based medications.”

The authors have analyzed how newly developed stem cell technologies could be used to treat PD, and how clinical researchers are moving very quickly to translate this technology to early clinical trials. In the past, most transplantation studies in PD used human cells from aborted embryos. While these transplants could survive and function for many years, there were scientific and ethical issues: fetal cells are in limited supply, and they are highly variable and hard to quality control. Only some patients benefited, and some developed side effects from the grafts, such as uncontrollable movements called dyskinesias.

Recent strides in stem cell technology mean that quality, consistency, activity, and safety can be assured, and that it is possible to grow essentially unlimited amounts of dopamine-producing nerve cells in the laboratory for transplantation. This approach is now rapidly moving into initial testing in clinical trials. The choice of starting material has also expanded with the availability of multiple human embryonic stem cell lines, as well as the possibilities for producing induced pluripotent cells, or neuronal cells from a patient’s own blood or skin cells. The first systematic clinical transplantation trials using pluripotent stem cells as donor tissue were initiated in Japan in 2018.

“We are moving into a very exciting era for stem cell therapy,” commented Dr. Parmar. “The first-generation cells are now being trialed and new advances in stem cell biology and genetic engineering promise even better cells and therapies in the future. There is a long road ahead in demonstrating how well stem cell-based reparative therapies will work, and much to understand about what, where, and how to deliver the cells, and to whom. But the massive strides in technology over recent years make it tempting to speculate that cell replacement may play an increasing role in alleviating at least the motor symptoms, if not others, in the decades to come.”

“With several research groups, including our own centers, quickly moving towards testing of stem cell therapies for PD, there is not only a drive to improve what is possible for our patients, but also a realization that our best chance is harmonizing efforts across groups,” added Dr. Henchcliffe. “Right now, we are just talking about the first logical step in using cell therapies in PD. Importantly, it could open the way to being able to engineer the cells to provide superior treatment, possibly using different types of cells to treat different symptoms of PD like movement problems and memory loss.”

“This approach to brain repair in PD definitely has major potential, and the coming two decades might also see even greater advances in stem cell engineering with stem cells that are tailor-made for specific patients or patient groups,” commented Patrik Brundin, MD, Ph.D., Van Andel Research Institute, Grand Rapids, MI, USA, and J. William Langston, MD, Stanford Udall Center, Department of Pathology, Stanford University, Palo Alto, CA, USA, Editors-in-Chief of the Journal of Parkinson’s Disease. “At the same time, there are several biological, practical, and commercial hurdles that need circumventing for this to become a routine therapy.”

Stem cell a boon to heal metastasis cancer

Researchers have made a new discovery about cancer cell metastasis that could completely change treatment approaches in the battle against cancer.

Sebastian Tomus | Shutterstock

Sebastian Tomus | Shutterstock

Professor Michael Lisanti from the University of Salford and colleagues have identified a rare rogue stem cell they are calling the “cell of origin” that could trigger the spread of cancer (metastasis).

“It feels like finding the proverbial needle in a haystack, and it crucially gives us a new window on cancer and how we might stop it.”

-Michael Lisanti, Lead Author

As reported in the journal Frontiers in Oncology, after growing cell lines derived from breast tumors, the team found that the most dynamic of the cells were particularly energetic.

Interestingly, those cells also displayed signs of senescence, the process that causes ageing cells to “freeze” and stop proliferating. However, the energetic cells appeared to have “broken out” of this process and lost their senescence, says Lisanti.

The researchers say there is a growing body of evidence suggesting that metastasis is caused by cancer stem cells that are fuelled by mitochondria. The team believes these “mother” cancer cells could use antioxidants and energy from mitochondria to redirect their fate.

“Scientists talk about cancer being caused by dying cells coming back to life, so-called ‘zombie-cells,’” says Lisanti. “We now see it is more dramatic than that…. this origin cell breaks out of line and runs amok, multiplying malignant cells and creating a tumor.”

However, most chemotherapy drugs are designed to target “bulk” cancer cells rather than these “root” stem cells; new therapies that specifically target these “tumor-initiating cells” are therefore needed to prevent the growth and spread of cancer.

Our new research may have discovered such a target,” said Lisanti and team. “Now that we have found them and we know how they behave, it should be relatively simple to find drugs to target cancer stem cells.”

The team adds that if they focus on energetic stem cells, they may be able to home in on the target directly and by doing so, revolutionize treatment and turn cancer into a manageable disease like diabetes.

We believe that we have arrived at the start of a new, more fruitful, road in cancer therapy. As a consequence, “big pharma” drug screening should actually focus on cancer stem cells and their relevant targets.”
