Puffing on electronic cigarettes can damage neural stem cells important to brain function, a new study says.

E-cigarettes produce a stress response in neural stem cells, researchers at the University of California at Riverside reported in a study published in the interdisciplinary open-access journal iScience.

E-cigarette users may think they’re safer and cleaner than tobacco cigarettes — but evidence is mounting that nicotine is harmful whether it’s smoked in a traditional cigarette or vaped in an e-cigarette. Another recent study found that certain e-cigarette flavorings damage cardiovascular cells.

Such products ‘are not harmless’, said Atena Zahedi, who earned her PhD in bio-engineering and co-authored the paper.

‘Even short-term exposure can stress cells in a manner that may lead, with chronic use, to cell death or disease. Our observations are likely to pertain to any product containing nicotine,’ Zahedi said in a statement.

E-cigarettes set off a complex series of cellular-level events that damage stem cells’ DNA, the researchers said.

‘The neural stem cells get damaged and could eventually die,’ Zahedi said. ‘If that happens, no more specialised cells — astrocytes and neurons, for example — can be produced from stem cells.’

Those cells are critical, the researchers said. Young people and foetuses are especially prone to stem cell damage because their brains are still developing, the researchers said.

That means young people and pregnant women could be particularly vulnerable to harm from e-cigarettes, the researchers say.

‘Their brains are in a critical developmental stage,’ said Prue Talbot, a professor in the Department of Molecular, Cell and Systems Biology who led the research and directs the Riverside campus’ Stem Cell Center.

‘Nicotine exposure during prenatal or adolescent development can affect the brain in multiple ways that may impair memory, learning, and cognition,’ Talbot said in the statement. ‘Furthermore, addiction and dependence on nicotine in youth are pressing concerns. It’s worth stressing that it is nicotine that is doing damage to neural stem cells and their mitochondria.’

Given the prevalence and availability of nicotine in liquid, inhabitable form, Talbot added, ‘We should be concerned about this.’

Drs. Wang and Buser: No. In the spine field, two other types of cells called osteoblasts and chondrocytes have been used. However, the clinical evidence and basic research on those cells are lacking. On the other hand, bone marrow aspirates that contain cells and growth factors are currently used in spinal fusion.

What types of spine problems are reportedly helped by stem cell therapy?

Drs. Wang and Buser: There is a lack of evidence and science to match a stem cell therapy to a specific spinal disorder. However, certain clinical trials and health care facilities are using stem cells to treat low back pain and disc degeneration, and during spinal fusion surgery.

Is stem cell therapy approved by the U.S. Food and Drug Administration (FDA)?

Drs. Wang and Buser: No. The evidence is not yet available to gain FDA approval.

Is the use of a patient’s own stem cells considered an off-label use of this biological technology?

Drs. Wang and Buser: For a product to be used off label, it must be FDA-approved for initial indication. As the use of patient’s own stem cell is not regulated by the FDA, it is unclear if it is considered off-label use. However, we are not aware of on-label uses of stem cells for spinal regeneration.

Without FDA approval, insurance will not cover the treatment. What may treatment cost? And, does treatment require multiple visits?

Drs. Wang and Buser: The costs vary, and there is no standardized protocol for the number of treatments because the clinical evidence is lacking. Other factors that affect cost and treatment are the specific spinal disorder and number of spinal levels impacted. Some patients might require one injection, and some might need several visits.

This augurs well for the authority to take up the Prime Minister’s appeal to the nation not to rely on present successes but to move forward towards innovative and challenging endeavours, such as was done in the case of blockchain and the introduction of cannabis for medicinal use.

Innovative initiatives need to be picked up in the early stages to lay a strong foundation for success to be endowed to the next generation, that is, for our children and grandchildren. These investments, which will bear fruit in the future, can only be carried out because in the last five years the government has set a strong economic and intellectual basis.

The authority is a prime example of an entity that has responded to the Prime Minister’s impetus and is moving towards becoming a sustainable and successful entity, especially through substantial investment in its team of highly committed and motivated professionals.

The assignment of the stem cell initiative to the Malta Medicines Authority, by the Secretariat for Consumer Rights, Public Cleansing and Support for the Capital City, was a move in the right direction, signalling a long but virtuous path towards innovation in advanced therapies, which is tomorrow’s science of medicine. Scientific innovation in stem cells cannot be conceived without advanced regulatory science structures and education.

This is something I have explained to the leaders of our tertiary education systems and it is good to see that there are already recommendations to the Senate of the University of Malta to move diligently but expediently in the field of the regulatory sciences.

Stem cell therapy is no longer science fiction. It is a challenge for the present and the future. Challenges present risks. The key to risk reduction in introducing stem cell therapy is the application of robust regulatory sciences.

This is what I am continuously encoura­ging the Malta Medicines Authority to execute, and I am fully confident that success will be achieved in this area. There are several barriers that still need to be overcome before this novel therapy can be translated from the laboratory to the clinic. But stem cell therapy has brought in optimistic hope among researchers, doctors and patients who are the chief beneficiaries of this innovation.

“We are in desperate need of a better way of helping people with PD. It is on the increase worldwide. There is still no cure, and medications only go part way to fully treat incoordination and movement problems,” explained co-authors Claire Henchcliffe, MD, DPhil, from the Department of Neurology, Weill Cornell Medical College, and Department of Neurosurgery, Memorial Sloan Kettering Cancer Center, New York, NY, USA; and Malin Parmar, Ph.D., from the Wallenberg Neuroscience Center and Lund Stem Cell Center, Lund University, Lund, Sweden. “If successful, using stem cells as a source of transplantable dopamine-producing nerve cells could revolutionize care of the PD patient in the future. A single surgery could potentially provide a transplant that would last throughout a patient’s lifespan, reducing or altogether avoiding the need for dopamine-based medications.”

The authors have analyzed how newly developed stem cell technologies could be used to treat PD, and how clinical researchers are moving very quickly to translate this technology to early clinical trials. In the past, most transplantation studies in PD used human cells from aborted embryos. While these transplants could survive and function for many years, there were scientific and ethical issues: fetal cells are in limited supply, and they are highly variable and hard to quality control. Only some patients benefited, and some developed side effects from the grafts, such as uncontrollable movements called dyskinesias.

Recent strides in stem cell technology mean that quality, consistency, activity, and safety can be assured, and that it is possible to grow essentially unlimited amounts of dopamine-producing nerve cells in the laboratory for transplantation. This approach is now rapidly moving into initial testing in clinical trials. The choice of starting material has also expanded with the availability of multiple human embryonic stem cell lines, as well as the possibilities for producing induced pluripotent cells, or neuronal cells from a patient’s own blood or skin cells. The first systematic clinical transplantation trials using pluripotent stem cells as donor tissue were initiated in Japan in 2018.

“We are moving into a very exciting era for stem cell therapy,” commented Dr. Parmar. “The first-generation cells are now being trialed and new advances in stem cell biology and genetic engineering promise even better cells and therapies in the future. There is a long road ahead in demonstrating how well stem cell-based reparative therapies will work, and much to understand about what, where, and how to deliver the cells, and to whom. But the massive strides in technology over recent years make it tempting to speculate that cell replacement may play an increasing role in alleviating at least the motor symptoms, if not others, in the decades to come.”

“With several research groups, including our own centers, quickly moving towards testing of stem cell therapies for PD, there is not only a drive to improve what is possible for our patients, but also a realization that our best chance is harmonizing efforts across groups,” added Dr. Henchcliffe. “Right now, we are just talking about the first logical step in using cell therapies in PD. Importantly, it could open the way to being able to engineer the cells to provide superior treatment, possibly using different types of cells to treat different symptoms of PD like movement problems and memory loss.”

“This approach to brain repair in PD definitely has major potential, and the coming two decades might also see even greater advances in stem cell engineering with stem cells that are tailor-made for specific patients or patient groups,” commented Patrik Brundin, MD, Ph.D., Van Andel Research Institute, Grand Rapids, MI, USA, and J. “Scientists talk about cancer being caused by dying cells coming back to life, so-called ‘zombie-cells,’” says Lisanti. “We now see it is more dramatic than that…. this origin cell breaks out of line and runs amok, multiplying malignant cells and creating a tumor.”

However, most chemotherapy drugs are designed to target “bulk” cancer cells rather than these “root” stem cells; new therapies that specifically target these “tumor-initiating cells” are therefore needed to prevent the growth and spread of cancer.

Our new research may have discovered such a target,” said Lisanti and team. “Now that we have found them and we know how they behave, it should be relatively simple to find drugs to target cancer stem cells.”

The team adds that if they focus on energetic stem cells, they may be able to home in on the target directly and by doing so, revolutionize treatment and turn cancer into a manageable disease like diabetes.
